Medical Office Force LLC. is a healthcare technology startup providing chronic care management and remote patient monitoring through its online and on phone platform, MyRemoteCheckup.
We’re expanding rapidly and looking for qualified candidates to join our team. As a NON-CLINICAL Patient Care Navigator, you will partner with our Client Independent Physician Practices in helping patients report their wellness levels between office visits and engage in their own health-care through our smartphone apps and phone interviews.
Through these ongoing relationships, we help patients prevent avoidable emergency room visits and hospital stays and feel closer to their doctors. You will work directly with chronically ill patients over the phone to deliver our program’s care management services, completing personalized care plans, setting health-related goals, and following up monthly to motivate them.
Day-to-day Responsibilities
Your duties will include, but are not limited to:
- Be an empathetic, compassionate, and active listener.
- Manage time effectively to achieve goals and think critically to solve problems.
- Establish relationships with and serve as the primary point of contact for patients.
- Make the required volume of outgoing calls daily, maintaining high efficiency and quality.
- Communicate with patients via phone, SMS texting, and email.
- Escalate patient requests and needs to our partner medical practices.
- Perform duties under strict compliance with HIPAA standards.
- Maintain documentation of all client encounters in the HWS Patient Portal.
- Knowledge of the medical field/EHR is preferred. Multilingual candidates (Spanish, Hindi, etc.) are highly valued.
Required Experience and Competencies
- High school diploma or GED required.
- Customer service experience (Call center or phone/online experience is a plus).
- Excellent keyboarding, computer, and online navigation skills.
- Strong verbal/written communication and phone etiquette.
- Ability to remain organized and manage competing priorities independently.
- Core Skills: Listening, Social Perceptiveness, Critical Thinking, and sound Judgment/Decision Making.

Hours & Travel
Full-time shifts covering 8 AM - 8 PM on a staggered basis, including nights and weekend coverage needs. Willingness to travel: 100% (Preferred).
